It is not really a scar, but April has a Kryptonian symbol permanently burned onto her right shoulderblade. A few years ago, April had been possessed by a Demon, and the only way to help her recover was for Clark (who at that time was only her friend) to use his heat vision and burn the symbol of love into her shoulderblade.
That same evening, they officially became boyfriend and girlfriend.
Later in their relationship, April was kidnapped by the Scourge. Clark rescued her, and she recovered. And during their first intimate moment, the love symbol magically turned into the Kryptonian word for soulmate.
